How to Fix MLB The Show 26 3v3 Mode Not Working Issue
There is nothing more frustrating than gathering your squad for some diamond action, loading up MLB The Show 26, and watching the 3v3 co-op mode completely fall apart. Whether you are staring at a frozen matchmaking screen, getting hit with the dreaded "Challenge Failed" pop-up, or getting booted straight back to the main menu before the first pitch is even thrown, you are not alone. Co-op bugs are an annual tradition, but most of them come down to server desynchronization, cross-play mismatches, or weird display conflicts.
Before you and your friends give up and go play solo ranked, try these community-tested workarounds. They cover everything from immediate squad fixes to quick system adjustments that will get your 3v3 matches running smoothly again.
Immediate Game & Console Fixes
Most 3v3 issues happen because the server gets confused about who is actually in the lobby or what state the party is in. Start with these basic fixes before messing with deeper settings:

Synchronize Relaunches: Do not just restart your own game. If your lobby bugs out or you get a "Challenge Failed" error, every single player in your pre-made squad needs to completely force close the game and restart it at the exact same time. This wipes out any ghost lobby states on the San Diego Studio (SDS) servers and gives your group a fresh slate.

Avoid Quick Resume: If you are playing on Xbox, Quick Resume is your worst enemy for online sports games. Launching MLB The Show 26 from a saved Quick Resume state completely breaks your live connection to the SDS servers, even if you look like you are online.
